FYI I use OSR2 2.1 here and at home both with FAT32.
A few things you can check. If your customer clicks on the oinsight.exe icon in the directory and he specifies Open Application what happens? Does he get an error? or does it open? If you run an application some of the options are greyed out.

Try creating a new shortcut using the /AP=APPNAME, UN=USERNAME /DV=1 option and see what happens there.
Otherwise, I go back to having them mail the engine to support and having them check the flags.
