Now this COULD be a really obscure bug we have been seeing quite a bit recently on AREV systems with modern OSs. Somehow (I believe incorrectly configured m/cs) the !FileName row gets deleted from the ! file. The only solution is to copy in a copy of the row or recreate it by removing and readding all indexes….

Only answer I found that works is removing and re-adding the offending index.
