Turns out this is a problem with Outlook where it decides to encode the email with extra RTF codes. We are currently experimenting with ways to turn this off using a Microsft KB. My original question was: is any known difference of the PDF attachment between sending a PDF via SMTP or via an email client using IMAPI (eg Outlook). I would prefer sending via SMTP as no email client is required.

There has been no problem with the attachment PDF.
