I have rebuilt system index.

What is happening here? How to fix it?

No, had to delete both to make backup to work w/o error msg.

How are they defined?

What steps do you use to create the entity?

Form Designer / Window Properties / Create Icon Entity as I have always done new icons. Is there also another way?

I used create icons in other applications but now it behaves same there too so I can not create a new icons anywhere.
