I had no problem creating the ICON entity from the Application Manager. I did have a problem in the Form Designer. The Form Designer created a BITMAP and ICON repository item. The BITMAP entity contains everything needed and the ICON entity is missing the actual pointer to the entity.

For now, create the entity from the Application Manager.
